المستندات المطلوبة لتنظيم محضر نقل جثمان إلى لبنان
- صورة عن بيان قيد عائلي للمتوفّي.
- صورة عن جواز سفر المتوفي .
- شهادة الوفاة الصادرة عن السلطات المختصّة والمصادقة عليها .
- شهادة من وزارة الصحّة تثبت أن الجثمان خالٍ من الأمراض المعدية .
- شهادة تثبت أنّه جرى تحنيط الجثمان بشكل كامل .
- جواز مرور للجثمان من البلد الذي كان يقيم فيه.
- الرسوم : 500 كرون سويدي .
ملاحظة : ينبغي أن يكون المتوفّي لبناني الجنسية أو فلسطيني لاجئ في لبنان.
Documents needed to transport a corpse to Lebanon
- A copy of the Lebanese family registration record of the deceased.
- A copy of the deceased’s passport..
- A death certificate issued by the competent authorities and duly legalized.
- A certificate from the Ministry of Health confirming that the body is free from any contagious or infectious diseases.
- A certificate confirming that the body has been fully embalmed.
- Laissez-Passer for the deceased from the country of residence.
- Fees: 500sek
Remark: The deceased should be Lebanese citizen or Palestinian refugee residing in Lebanon.
